SUMMARY:

Performs healthcare coordination and case management activities IAW Medical Treatment Facility (MTF) instructions.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Clinical social worker may be required to function in a specialty mental health clinic or in a patient centered medical home (PCMH) location as a behavioral health consultant (BHC) and/or Behavioral Health Care Facilitator (BHCF). The type of work may include, but is not limited to the following:

  • Maintain accurate and current notes in the Mental Health records, inpatient medical records and electronic medical records of all patients seen, as appropriate, and produces reports of evaluation and/or treatment, as required.
  • Participate in military specific training (e.g., training to perform security clearances, use of the Air Force Guide to Managing Suicidal Behavior, pre-post deployment screening, PTSD, and combat stress) and others as directed by the mental health flight Commander or AF policy.
  • Provide brief (typically no more than four appointments) cognitive/behavioral interventions in primary care under the Air Force consultation model (i.e., per the Air Force Behavioral Health Optimization Program Practice Manual). Services will mimic the pace of primary care (i.e., patients seen for 15- to 30-minute appointments).
  • May provide classes and/or groups in format appropriate for primary care (e.g., drop-in stress management class, group medical visit for a chronic condition).
  • Consult with PCMs on-demand about general issues or specific patients.
  • Ensure PCMs receive same-day verbal and/or written feedback on patients referred.
  • Deliver brief presentations/trainings to primary care team members regarding behavioral health issues and BHC services.
  • Participate in team efforts to develop, implement, evaluate, and revise clinical pathway programs needed in the clinic.
  • Supervision of providers who are in training or not fully licensed.
  • Consult with medical personnel, legal authorities, and military commanders as required.
  • Contact referred patients to assess treatment adherence.
  • Use validated symptom inventories to assess current symptom severity.
  • Assess barriers to treatment adherence and help patient problem-solve solutions to barriers.
  • Provide patients with education regarding their mental health condition and treatment regimen.
  • Routinely staffs difficult cases with the referring provider, behavioral health consultant and/or psychiatry consultant.
  • Perform case management functions.
  • Attend and participate in meetings during normal duty hours, professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, mental health, flight meetings, and other meetings required by applicable regulations, MTF guidance, or as directed by the mental health flight Commander or his/her designated representative.
  • Responsible for and applies psychological procedures and techniques, including interviewing, behavioral assessment, and evidenced-based therapies, in the evaluation, diagnosis, and treatment of psychological and neuropsychological disorders using the following skills: individual, family and group psychotherapy, and couples therapy, alcohol and drug treatment evaluations.
  • Consult with medical personnel, legal authorities, military commanders, and school districts as required.
  • Maintain regular and punctual attendance.
  • Perform other duties as assigned.

SUPERVISORY RESPONSIBILITIES:

May include supervision of unlicensed providers or trainees.

Job Requirements

MINIMUM QUALIFICATIONS:

  • Must hold a Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Maintain a current, unrestricted clinical license to independently practice social work in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
  • Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in a mental health setting working with an adult population, or with children and teens if specifically working with that population. Experience working in a government setting such a DOD or Department of Veterans Affairs (VA) MTF is preferable.
  • Must have experience in brief behavioral interventions. Demonstrates knowledge, skills, and ability to use basic office automation and information management equipment and programs (i.e., word processing, database management, email, etc.).
  • Must obtain and maintain any necessary security access and/or background checks.
  • Must follow site required health screening and immunization requirements.

DESIRED QUALIFICATIONS & SKILLS:

  • Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
  • Excellent oral communication skills for patient education and telephone management of patients.
  • Sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients and healthcare staff from diverse backgrounds.
  • Ability to effectively read, write and speak English in order to communicate with all patients and other healthcare providers.
  • Ability to effectively communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.

REQUIRED TRAINING:

Participate in and successfully complete Air Force BHC training. Demonstrates core competency skills to an expert trainer within 90 days of starting to see patients in primary care.

PHYSICAL DEMANDS:

Position consists of sitting for long periods of time, using hands/fingers, handling, reaching with hands and arms, speaking or translating. Periods of walking, standing, bending, kneeling, stooping, crouching, and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. May be assigned occasional to moderate periods of analytical tasks requiring extensive reasoning abilities. WORK SCHEDULE:

40 hours per week Monday through Friday. May be required to work additional hours as needed to complete assignment or project.

WORK ENVIRONMENT:

Work is performed primarily indoors at a well-lit military treatment facility such as a clinic, outpatient facility or hospital. The environment is normally air conditioned, but conditions may change dependent upon circumstances. Work may need to be performed in a fast-paced environment requiring quick thinking and rapid judgements. Employee will be exposed to a wide variety of clients in differing functions, personalities and abilities. Exposure to bloodborne pathogens and bodily fluids, latex, radiation or other contaminants and safety issues. May be exposed to infection or diseases from contact with patients. Work is highly social with patients and can be stressful with a high emotional element. Decisions made may substantially impact patients and their families.

COMPENSATION & BENEFITS PACKAGE

  • Pay is: $29.00 to $33.41per hour
